And fill out the AdobeSign agreements which pop up. The links above contains (3) Documents:
Master Rental Agreement – This lays out the the terms and conditions for rentals with Central.
Project / Rental Authorization – This authorizes us to hold equipment on your account for a specific project or event (one of theses will be filled out each time you rent with us).
Credit Card Authorization – This will be used as the form of payment for your first rental, and to secure any subsequent future rentals on the account. NOTE – You will either need to stop by our office to let us verify the card in person after filling out the authorization, our provide us with a copy of the card via email or fax to verify.
After you fill out the forms above the last step for the new account is to provide us with a Certificate of Insurance for corporate accounts or Proof of Insurnace / 3rd Party Rental Insurance for personal accounts before equipment can leave our premises.
Corporate customers must have a current “ACORD” formatted certificate of insurance issued to Central Planning, LLC – DBA Central Rentals, naming Central Planning, LLC as an additionally insured, showing at least the minimum coverages below:
Comprehensive General Liability:
Must be in an amount of no less than $1,000,000 / $1,000,000 bodily injury liability$1,000,000 property damage liability.
Auto Liability:
Must be in an amount of no less than $1,000,000 / $1,000,000 bodily injury liability$1,000,000 property damage liability.
Auto Comprehensive & Physical Damage:
Must name Central Planning as Loss Payee for comprehensive and physical damage to rented vehicles equaling or exceeding the replacement value of the rented vehicles. Coverage must be noted on the top sheet of your Accord certificate in addition to evidence of Auto Liability.
Misc. Rented Equipment:
Limits must equal or exceed the replacement value of rented equipment.
